University of Toledo Immunization Compliance Report
The University of Toledo faculty, staff and students can quickly review their immunization history and requirements. Notifications are sent when requirements are out of compliance or due within the next 30 days. Copies of the compliance report can be emailed. A valid UTAD account is required.
